It's a vicious circle once your behind in these contests. You risk more and generally because of that you fall behind even further, meaning you have to risk even more in the next round.
I also made a couple of bad mistakes this year. Missing the deadline for two matches was an obvious one
Another one was picking Dott in his last 16 match just because I wanted to see a Ronnie v Dott quarter. Once you start with the wishful thinking picks it's as good as over.

It's happened only once before in 2007 when four matches finished 13-8
We've had a few occasions where we've had three scores the same with one match being very close, namely 2002 when we had three 13-3 and two 13-2s
